Otherwise, please follow the instructions below:

INSTRUCTIONS: All fields are necessary unless otherwise specified. You must complete the entire application in one sitting, as the application cannot be saved. 
The form is sectioned into a few parts:

Part 1.1 consists of information pertaining to your visa application; please double check the information is correct.
Part 1.2 consists of your flight information, A travel ticket must be booked by the time you submit this form.  Please also note that we can only reimburse for Economy class flight ticket. 
Part 1.3 consists of general information about your affiliation/contact information. 
Part 2.1 consists of information regarding accommodations and misc.
