Congratulations again on your acceptance into the 2015 Shanghai Neuroeconomics Collective Summer School!

The next step in your admission is to start your visa application process. To obtain a Chinese visa, you will need to visit a Chinese Embassy and submit the required documentations yourself. NYU Shanghai will provide you with the letter of invitation as one of the required documents, but in order to do so, we need you to fill out the following form.
 
Before you start this form, please make sure that your passport is valid for at least another 6 months; otherwise, you must get your passport renewed. 

To ensure that we  have enough time to issue you a letter for the visa, please complete this form by 11:59 pm (EST) of Sunday, March 22th, 2015. 

***If you already have a valid Chinese Visa, please write 'N/A' in sections of the form that ask for passport and/or flight  information.
